If you have not received a response within 10 business days, please send a follow-up email referencing your original request date.
For questions regarding outstanding balances, insurance claims, or billing statements, please use the same email address above and write "Billing Inquiry" in the subject line. Include your full name, date of birth, and a brief description of your question.
Please note that billing inquiries related to active insurance claims may require additional time to process depending on the status of your claim with your insurance provider.
