After 30 years of serving patients throughout the Phoenix Metro and Tucson communities, Chiro+Plus Chiropractic Centers and First Medical+Plus have closed their doors.

We are grateful to all our patients and business associates for choosing our clinics for your chiropractic and medical needs. It was a privilege caring for you and we wish you continued health and happiness. To locate a new chiropractic or medical provider we recommend contacting your Health Insurance Company to get a list of providers in your area. As always, if you are experiencing an emergency, please call 911 or go to your nearest hospital emergency department for evaluation and treatment.

What to Expect

Once we receive your request, our team will verify your identity and locate your records. You will receive a confirmation reply acknowledging your request. Records will be sent to the email address provided in your request.

If you have not received a response within 10 business days, please send a follow-up email referencing your original request date.

Billing Inquiries

For questions regarding outstanding balances, insurance claims, or billing statements, please use the same email address above and write "Billing Inquiry" in the subject line. Include your full name, date of birth, and a brief description of your question.

Please note that billing inquiries related to active insurance claims may require additional time to process depending on the status of your claim with your insurance provider.

  • Records requests are processed in the order they are received.
  • We are unable to release records to third parties without a signed authorization form.
  • Records may be provided in digital format (PDF) via secure email.
  • There is no fee for standard medical records requests.
  • Requests for X-rays or imaging studies may require additional processing time.
